Instrumenting the workflow told a different story: 79% of summaries were substantially edited before use, and the edits clustered in exactly two clauses the agent consistently misread.

The fix took a fortnight once it was visible. The point is that it had been invisible for eight months, and every dashboard said the agent was working.
